| 2. | At the CCS Customer Logon page, enter your account number and password, and then click Login. |
| 3. | Click Schedule D or Form 1099 at the top of your account page. |
| 4. | To print Schedule D, click the printer icon on your browser. To print Form 1099, click the printer icon on the webpage generated by Adobe Acrobat. |
